It got its name from the Otoe Native American tribe, who once populated the area. The county seat is Nebraska City, which also serves as the county's largest city and a principal hub of activity.

In the mid-1800s, Otoe County was an essential part of pioneering life, and Nebraska City played a significant role as a gateway to the West due to its position along the Missouri River. The city was founded in 1854 by the Nebraska City Company, and it later became a critical transportation and trade center as people ventured westward. The Oregon, California, and Mormon Pioneer Trails, which were crucial passageways during westward expansion in the United States, intersect in the county, further underlining its importance to the nation's history.
